Mechanisms of action
After consumption, probiotics produce a special metabolite called butyrate in the intestine. Butyrate produces protein, through some cells in the bone marrow, which is effective in bone growth. The volume of spongy bones in the bone increases after the consumption of probiotic supplements and the production of butyrate in the intestine. Probiotics also reduce the possibility of osteoporosis by increasing bone volume and bone growth.
Probiotic bacteria reduce inflammatory cytokines by modulating intestinal immune responses and affecting the intestinal lumen which increases the absorption of minerals like calcium.
Also, bacterial secretory factors are transferred to the blood stream and then to the bone. They can communicate with osteoclasts and osteoblasts as well as immune cells leading to reduce the expression of pro-inflammatory and osteoclastogenic cytokines and oxidative stress and increase the accumulation of minerals and the expression of proteins which result in decreasing osteoclast formation and increasing bone density.
Bifidobacterium species also produce short chain fatty acids that can lower the pH of the intestinal system and subsequently increase the absorption of minerals.
Side effects
All probiotics exist naturally in the body and all are registered as GRAS which means safe, by the US Food and Drug Administration, and no specific side effects regarding toxicity have been reported for probiotics. In small cases, consumption of this product may lead to bloating and gas production increase in the digestive system which will disappear after continued use. Clinical trials
Hamid Y. Dar et el. (2018) investigated the effect of lactobacillus acidophilus strain in ovariectomized rats. In a study and stated that Lactobacillus acidophilus in ovariectomized rats causes an increase in trabecular spongy and cortical bones along with an increase in the density of mineral substances and maintaining the heterogeneous state of the bone. This effect of Lactobacillus acidophilus strain follows the modulation of the host’s immune system.
Sarika Amdekar at el. (2012) investigated the effect of lactobacillus acidophilus strain on bone health and arthritis in a study. For this purpose, 30 eight-week-old Wistar rats were treated, 30 Wistar rats were treated for eight weeks, and after 21 days, the results showed that oral consumption of Lactobacillus acidophilus strain, both alone and in combination, reduced bone damage and stimulated the anti-arthritic and antioxidant activities against inflammation of joints in mice.
Ahmad Gholami at el. (2020) investigated the effect of probiotic consumption on bone loss in type 2 diabetes patients taking pioglitazone in a study. Streptozotocin (60 mg/kg) was administered to induce diabetes and then, the diabetic rats were fed orally with pioglitazone (300 mg/kg) and probiotics (1×109 CFU/ml per day), alone and in combination, for four weeks. Simultaneous treatment of probiotics with pioglitazone significantly reduced urinary calcium, creatinine and the ALP. Pioglitazone was shown to have harmful effects on the femur BMD while treatment with probiotics significantly subsided these effects. Among the tested probiotics, Bifidobacterium lactis was shown to have the best protective effects in pioglitazone-induced bone loss in diabetic rats.
